Notes:

1. The dental examinations of BLOOM (Dental) program are conducted at Humansa Dental (Unit 505, 5/F, New World Tower I, 16-18 Queen's Road, Central, Hong Kong).
2. The eye examinations of BLOOM (Eye) program or optional add-ons are conducted at Humansa Eye (Unit 803, L8, K11 ATELIER Victoria Dockside, 18 Salisbury Road, Tsim Sha Tsui, Kowloon, Hong Kong).
3. The above prices are effective as of April 2023. Humansa reserves the right to adjust the prices without notice.
4. Dental X-ray will be administered upon the result of dental check-up.
5. Imaging screening is at the parent’s discretion.
6. Stool sample is collected at home.
7. Fasting is required at least 8 hours prior to examination if adding on whole abdominal ultrasound.

2. Bloom Children Health Check fee includes either dental assessment or eye examination. Any optional add-on examination items must be purchased at their original price.
3. Advance booking is required. Please reach Humansa by clicking the direct WhatsApp enquiry link in the description at the time of booking for an appointment. No walk-in services would be available.
4. Cancellation or rescheduling of appointments must be made through WhatsApp or phone call at least 48 hours after the booking is confirmed. In the event of a no-show on the scheduled health check day, refunds will not be accepted.
5. The health check program offer is subject to availability and will be provided on a first-come, first-served basis.
6. The offers cannot be used in conjunction with other promotional offers; it cannot be exchangeable for cash or other product. The offer is non-transferrable and non-refundable.
7. Chinese version of these Terms and Conditions are for reference only. In case of discrepancies between the provisions of English version and the Chinese version, the English version shall prevail.
