2. FOUNDATION health check program fee includes one-time initial consultation and one-time follow-up consultation on medical report analysis . Any additional follow-up consultations, treatments or medications are not included in the program.
3. Any optional add-on examination items that are not included in the program must be purchased at their original price.
4. Advance booking is required. Please specify the health check program and promotional code at the time of booking and contact Humansa | Victoria Dockside at (+852) 3180 2288 or WhatsApp: (+852) 6179 3887 for an appointment. No walk-in services would be available.
5. Cancellation or rescheduling of appointments must be made through WhatsApp or phone call at least 48 hours after the booking is confirmed. In the event of a no-show on the scheduled health check day, refunds will not be accepted.
6. The health check program offer is subject to availability and will be provided on a first-come, first-served basis.
7. The offers cannot be used in conjunction with other promotional offers; it cannot be exchangeable for cash or other product. The offer is non-transferrable and non-refundable.
8. Chinese version of these Terms and Conditions are for reference only. In case of discrepancies between the provisions of English version and the Chinese version, the English version shall prevail.
